APDA Financial Hardship Fund
APDA Wisconsin Chapter has a limited amount of funds available to assist people with Parkinson’s disease (PWP) who are experiencing financial difficulties.
Approved applicants can receive $400 every twelve (12) months. This fund is intended to help improve the quality of life of a person with Parkinson’s.
To be considered for an award, the applicant must:
- Reside in Wisconsin
- Have a diagnosis of PD/Parkinsonism
- Be experiencing financial difficulties
- Understand that this award is not intended as an ongoing support fund
- Complete a post-experience survey
- Reside in the community, not in a rehabilitation center, long-term care, or skilled nursing facility
- Understand that the payment may take at least 30 days to process after the application is approved
- Further eligibility guidelines are on the application.
Applications are reviewed on a rolling basis. Awards are made on a first-come basis and are subject to application approval and the availability of funds. The fund is subject to change or discontinuation with limited notice.
After your application is submitted, APDA will review your information and contact you to discuss your request. We will also work with you to identify any additional or alternative resources that may be available to support your needs.
